VCU School of Medicine Forms and Templates
The following Sensitive Data Sharing Agreement is designed for the use by VCU School of Medicine personnel in regard to the sharing of sensitive data with non-School of Medicine departments within the Virginia Commonwealth University or external affiliates of the University.
The following Remote Access Request form is designed for the use by VCU School of Medicine personnel in requesting remote file access from offsite locations.
The following IronKey User's Agreement form is designed for the use by VCU School of Medicine personnel in requesting an IronKey 4 GB Secure USB drive. The IronKey USB drives are encrypted USB / flash drives that are compatible with both Windows and Mac OS. This device should be used to securely transfer sensitive, confidential and protected data by all VCU School of Medicine employees. This form can be completed by any VCU School of Medicine employee with needs to transfer sensitive, confidential and protected data using USB / flash drives. This request must be approved by the employee's department.
The following Policy and Standards Exception Request form is designed for the use by VCU School of Medicine personnel in requesting exceptions to the VCU School of Medicine Information Security Standards.
VCU Forms and Requests
- Request a vulnerability scan for your server
A vulnerability scan should be performed on a server periodically to ensure the adequacy of protection on the server. Vulnerability scans can help identify security issues related to the server that may not have been noticed before. The University Security Team currently offers vulnerability scans on departmental and school servers upon request. The vulnerability scans can be directly requested from the VCU Information Security Page. Similar to vulnerability scans on a server, a web application should also be scanned for potential security flaws prior to entering the production environment. A verified and secured web application minimizes the risk of compromise of the web server, as well as the web applications. The vulnerability scans can be requested from the VCU Information Security Page. You may also contact the VCU SOM Information Security to submit a request.
Lost or Stolen equipment Reporting
As required by the VCU Information Security Standard, any lost or stolen computers, or loss of storage devices and electronic media with sensitive information must be reported to the University Information Security Department within 24 hours. Several methods exist for reporting of a lost or stolen device:
