All other transcript requests including Nursing, Education, etc., must contact VCU Records and Registration at 804-828-1349.
VCU medical student and alumni transcripts are maintained by the School of Medicine Registrar’s Office.

Official and unofficial transcripts may be requested by submitting in writing, either by letter or by using the Transcript Request Form [PDF]. Please mail letter or form with a check made payable to VCU SCHOOL OF MEDICINE to Heather C. Davison at the above address.
VCU School of Medicine requires the signature of the student or alumnus requesting his/her transcript.
