Candidacy/IRB IUCUC Compliance
The Graduate School and the School of Medicine now require an indication that students are aware of regulations governing the use of Human Subjects and Animals in research. The School of Medicine has a further requirement to ensure that should the research conducted by the student under the direction/counsel of a member of the faculty involve either Human Subjects or Animals that the advisor provide documentation that the student is approved for work on the project in question.
Completion of the Graduate School form and the School of Medicine form which appear below would normally be done as the student initiates work under the direction of her/his permanent advisor. Note that if IUCUC or IRB approved projects are involved the advisor must provide documentation validating the participation of the student in the project.

A copy of the Graduate School “Candidacy” form and a copy of the School of Medicine IRB / IUCUC Compliance form (and, if appropriate the required supporting documentation) must be provided to the School of Medicine Office of Graduate Education as the permanent advisor is identified.
